List of documents required to get a work permit from GIRFO – Ukraine State Department on the Cases of Citizenship, Immigration and Registration of Physical Entities

From the applicant

1. Copy of the first pages of passport with notarized translation;

2. Certificate of absence of criminal record in Ukraine;

3. Medical document confirming negative test on AIDS and tuberculosis;

4. Apartment purchase agreement or apartment lease agreement registered with housing

Agency (ZHEK, etc.);

4. In case of apartment lease – a certificate (“Form # 3”) and a consent (certified by the housing agency, such as ZHEK) of all adult family members (cohabitants) that they do not object foreigner’s temporary registration in the apartment;

5. Copy of the insurance policy (certificate) covering the emergency medical treatment;

6. 6 black and white photos sized 3*4 cm;

7. Receipt for payment of the state duty and GIRFO services;

8. 4 envelopes and plastic folder;

9. Ukrainian State Tax Service Certificate of payment of all taxes.

From the company, which invites applicant to work

1. Notarized copy of work authorization letter;

2. Copy of the applicant’s tax ID number (you need to undergo a special procedure of obtaining a valid Ukrainian tax ID number);

3. Copy of the Certificate on the registration of company, which invites applicant to work;

4. Notarized copy of the Statistics committee Certificate of the company, which invites applicant to work;

5. Certificate from the bank certifying that the bank account of the company, which invites applicant to work, is active;

6. Copy of the company’s, which invites applicant to work, registration card with GIRFO in Kyiv.
